Can Nepal replace the lost maternal outreach before complications and deaths become invisible again?
Community workers connect poor households with care and produce the information needed to identify preventable deaths.
What we currently think
A replacement has not been demonstrated in Rautahat; the investigation describes disrupted visits within an already fragile system.

In Nepal, the visit that stopped coming
Kabita Mukhiya died in December 2025, aged 20 and pregnant. Her death is not this week’s event: Kristen Gelineau and Bram Janssen’s AP investigation, published on August 13, reconstructs what happened. A nurse had recommended a blood test whose importance she did not understand. After her baby died, one hospital referred her elsewhere; without the money or an understanding of the danger, she went home. AP reports eclampsia compounded by severe anaemia. Community visits that explained warning signs and accompanied women to appointments had stopped after US funding cuts.

- USAID fallout: In Nepal, when the aid workers stopped coming, the women and babies started dying
AP reports prenatal consultations halving, five neonatal deaths at one centre after a year with none recorded, and a discrepancy between two infant deaths in public records and eight counted by a partner. These local findings have no second independent count here and do not establish national deaths caused by the cuts.
- CARE awarded USAID program to strengthen adolescent reproductive health in Nepal
CARE announces a $35 million agreement effective June 1, 2022, with completion estimated for May 31, 2027. This announcement measures no health outcomes.
- Motherhood in the Madhes
This March 2026 local reporting documents municipal maternity initiatives alongside funding, equipment and hospital-referral problems. Its date does not establish what conditions were before the 2025 US cuts.
- Request for Proposal for Services LRPS-2026-9204163: To support the implementation of Integrated Management of Acute Malnutrition Program and Lactation Management Units
UNICEF’s procurement document describes low wasting-treatment coverage, inactive community workers, irregular screening, weak records and incomplete follow-up of dropouts. It is not a causal mortality study.
